- Fill the application form carefully and affix a recent passport-size photograph.
- Attach self-attested copies of all required documents, certificates, and experience letters.
- Prepare a Demand Draft of ₹500/- (if applicable) in favour of “ESI Fund Account No.2” payable at Delhi.
- Send the duly filled application in a cover super-scribed “Application for the post of Professor for Medical Institutions” by Speed Post to the address given in the notification on or before the last date.

Age Limit
Not exceeding 50 years (relaxable by 5 years for ESIC/Govt employees)
Selection Process
Selection will be made on the basis of Interview conducted by the Selection Board.
